Reservations Form not Working

  I tried the reservations page on your demo and it works fine. Yet I attached it to my site. Same parameters as your original design and it will not give me a successfully sent or error. Nothing. It stays on the same screen and doesn’t go anywhere. At first it highlighted the name phone and email fields. So I went  to your demo and it works there. Help please.
